GUIDE Can Help: Support for Dementia Caregivers

Caring for someone with dementia can be both rewarding and overwhelming. If you or a loved one has recently been diagnosed with dementia, you may now be eligible for a new Medicare benefit designed to support you on this journey.
The GUIDE Model (Guiding an Improved Dementia Experience) was created to improve the quality of life for people living with dementia and to reduce the strain on their unpaid caregivers.
What GUIDE Offers:
  • Free Respite Care – Relief for caregivers so they can rest and recharge.
  • 24/7 Hotline Access – Support is always available, day or night.
  • Care Coordination – Guidance in navigating healthcare and community resources.
  • Education & Support – Helping families better understand dementia and how to manage care.
Simple Eligibility Requirements:
  • Must have Traditional Medicare Insurance.
  • Cannot be enrolled in a Medicare Advantage Plan.
  • Must have medical documentation of dementia or an Alzheimer’s diagnosis.
  • Must not be a resident of a long-term care facility, hospice, or PACE program.
