We are proud to partner with Healthspan to offer the GUIDE Model — an innovative dementia care program developed by the Centers for Medicare & Medicaid Services (CMS).
The GUIDE Model (Guiding an Improved Dementia Experience) is designed to provide comprehensive, coordinated care for individuals living with dementia while offering meaningful support to their caregivers.
What Is the GUIDE Program?
The GUIDE Model is a nationwide initiative created by CMS to improve quality of life for people living with dementia. The program focuses on:
- Personalized care planning
- Care coordination across providers
- Medication management oversight
- Caregiver education and training
- 24/7 access to support
- Respite care services
This model is designed to help families navigate dementia with greater confidence, clarity, and support.
Our Partnership with Healthspan
Through our partnership with Healthspan, we are able to deliver enhanced dementia-focused care coordination and access to GUIDE benefits.
Healthspan works alongside our local team to:
- Conduct comprehensive assessments
- Develop individualized care plans
- Provide ongoing care navigation
- Connect families to community resources
- Support caregivers through education and respite options
Together, our goal is to reduce caregiver burnout, prevent unnecessary hospitalizations, and improve overall well-being for both patients and families.
Who Is Eligible?
To qualify for the GUIDE Program, individuals must:
- Have a documented dementia diagnosis
- Have Traditional Medicare (not a Medicare Advantage plan)
- Not be currently enrolled in hospice

The Power of Respite Care
One of the most valuable benefits of the GUIDE Program is respite care, which provides caregivers with much-needed relief.
Respite care may include:
- In-home caregiver support
- Adult day center services
- Short-term 24/7 skilled nursing facility stays
This support allows caregivers time to rest, attend appointments, or simply recharge — knowing their loved one is in safe hands. Respite may only be utilized at GUIDE approved facilities.
